Minimum Payment Policy Change
Effective August 1, 2010, outstanding balances less than $20 should be paid in full. The minimum monthly payment
on a balance of up to $200 is $20. Balances in excess of $200 require a minimum monthly payment of 10 percent
of the balance.

No Show Policy Change
Effective Aug. 1st we will start charging $25 for the 2nd missed apppointment. Charges for additional missed
appointments will be as follows: $50 for the 3rd missed appt., $75 for the 4th missed appt., and $100 for the 5th and further missed appointments.

Photo ID Required
Effective 11/1/2009, Priority Care Pediatrics requires valid photo identification issued by a local, state, or federal government
agency. This is a requirement mandated by the Federal Trade Commission (FTC) as part of the Red Flags Rule. Please
