At Priority Care Pediatrics, our Patient Care Coordination (PCC) Team  supports children with ongoing or complex medical conditions by helping families navigate care, treatments, and follow-up needs. This dedicated team — made up of nurses and care coordinators — works closely with your child’s primary care provider to ensure every aspect of care is well-organized, connected, and easy to manage.

We assist with specialist communication, medication tracking, hospital follow-up, and care plans, helping families feel confident that nothing falls through the cracks. By keeping your pediatrician and our care team closely aligned, we provide seamless, family-centered support for children who need extra coordination and attention.
